Launching a new operations structure in Bristol Adult Social Care

An important update about the next phase of the transformation journey in Bristol Adult Social Care

From Monday 29 September, we will begin implementing our new Target Operating Model (TOM), a significant step forward in our efforts to improve outcomes for the people we support, strengthen our partnerships, and ensure our services are sustainable.

What is changing?

The new operations structure has been shaped through 18 months of extensive engagement with colleagues and reflects our shared ambition to deliver high-quality, person-centred care. It also supports more effective collaboration between commissioning and operational teams.

As part of this change:

  • Our ‘front-door’ service has evolved into a multi-disciplinary team based at City Hall, now called the Advice, Information and Assessment Service.

  • Contact details remain the same - the online portal and Care Direct number: 0117 922 2700 are unchanged.

  • We are also aligning our processes with the new structure. This begins with safeguarding and first point of contact processes, followed by reviews of our community and specialist teams.

  • Finally, I’d like to introduce our Heads of Service and Service Managers in the new operational structure:

A shared effort

This transformation is about more than a new operations structure. It’s about culture, collaboration, and clarity; making it easier for teams to work together, for people to navigate our services, and for us to deliver on our commitments.

Please feel free to share this message with your colleagues to ensure everyone is informed and supported.

If you have any questions or feedback, or would like to discuss the TOM work further, please contact either the Transformation Team at: ASC.Information@bristol.gov.uk or contact me directly.
